Maryland Medical Assistance applications are processed through local Departments of Social Services. The Carroll County Department of Social Services is located at 10 Distillery Drive in Westminster, Maryland. To arrange for an appointment, please call (410) 386-3300.
Maryland Primary Adult Care Program applications are available through the previous link or by calling the Carroll County Core Service Agency at (410) 876-4440. Applications must be mailed to PAC Program Application, P.O. Box 386, Baltimore, MD 21203-0386 to be considered. Eligibility is usually established in about 30 days. More information may be obtained by calling 1-800-226-2142.
Most services must be pre-authorized by MAPS-MD or the Carroll County Core Service Agency. It is the responsibility of the service provider to obtain necessary authorizations. People seeking traditional outpatient therapy may simply arrange for an intake appointment with a service provider, who will get the necessary authorizations.
